The above is an example of the information to be contained in your Annual Training Log that you maintain in a book like format, however any record keeping method can be used.

Black Belt participating in the rank advancement program must use this log to keep track of their annual training hours.  

If participating your training logs are subject to review and a copy of this log must be submitted annually along with your annual registration application to determine eligibility for promotion.

Instructors maintain training records for their students, however I would make your students, under your supervision,  keep a training log book of their own.

A personal note:  

I would not skimp here - especially if your just getting started. 

Go buy yourself a very nice leather-bound book with blank pages and keep very neat and articulate notes.  Maybe even include some hand drawings or sketches and maybe even put a little color into it.

The only thing that really separates the ancient martial arts masters from modern ones is time and the records they kept and left behind.  And who knows, maybe in 300 years someone will have your book encased in some temple somewhere and referring to your works as "The Classics"    And if not, well its still something very nice to leave behind for your children as you found your own lineage.

And if you already have many years of experience but haven't yet written down anything its time you got started.   Basically your writing your own book here which will one day turn into your Masters thesis.
